A truck driver involved in a fatal five-vehicle collision on Monday morning may face charges says the Florida Highway Patrol.

It happened around 6 a.m., on U.S. 27 in Lake County, Florida.

According to reports, 43-year-old Tyrone Ivy was stopped at a stop sign on eastbound Indepence Boulevard in heavy fog. He then attempted to make a left turn onto northbound U.S.27 when his trailer was struck by several vehicles that were traveling south.

In total, four vehicles struck his rig. One of the drivers, 21-year-old Kevin Cottle died when his pickup went underneath the trailer.

Four others were hospitalized; two of which suffered serious injuries.

The Florida Highway Patrol said fog was a factor and are currently investigating to determine if charges will be filed. They said they’ll check if the truck was using proper lights and reflectors, if it crossed the intersection too soon, and if the driver exceeded his legal driving hours.

“We’ve got to make sure that that truck driver was following all the rules as far as time’s concerned,” said FHP’s Sgt. Kim Montes.
