A crash turned fatal when a van carrying high school water polo players collided with a grain truck.

According to the Henry County Sheriff’s Office, the incident occurred on Friday around 10 a.m. near State Route 65 and State Route 281 in Toledo, Ohio.

A passenger van, driven by Vicki Fisher, was carrying the players and family members from Worthington Kilbourne High School.

They were headed to a tournament in Napoleon.

A preliminary investigation found that 23-year-old Dillin Chiow was driving a semi truck south on State Route 65. At the same time, van driver Vicki Fisher was traveling west on State Route 281. NBC 4i reported that Fisher stopped at a stop sign but then failed to yield to the semi truck with the right-of-way.  The van pulled out right in front of the semi and was hit on its passenger side.

The force of the impact sent both vehicles crashing into a ditch.

19-year-old Courtney Fisher was pronounced dead at the scene.

Courtney’s mother (Vicki Fisher), and Courtney’s sisters Betsy, age 17, and Melanie, age 14, along with 15-year-old Samantha Fink and 14-year-old Sydney Zullick, were transported by helicopter with life-threatening injuries.

Worthington Schools’ superintendent Trent Bowers commented on the incident. “This is a really tough day for Worthington schools. We care deeply about our kids. These are great families. And we lost a really special young lady today,” he said.

The truck driver, Dillin Chiow of Anna, Ohio, was not physically injured in the crash.

According to 10tv, in the past eight days, there have been 3 crashes and two deaths in that intersection. Ongoing construction has caused an unusual amount traffic on those roads. Despite all the warning signs, stop signs and flashing lights placed by ODOT on Route 281, serious accidents have still occurred.

ODOT has placed additional warning signs along the road and are considering turning the intersection into a four-way stop.
